Completely this form is NOT an Order Confirmation. For various reason Seared Vision does not accept every commission offer. If your offer is declined you will be provided a reason. There is a deposit fee of $15 CAD for any accepted order. Deposit must be paid before design job begins. Failure to communicate can result in a returned deposit. Shipping costs will be covered by customer.
